I'd be happy to provide it, if anyone is looking. I charge 1% of the HIT pay, for 100%, no deductible coverage, if you have a 99% Approval rating. If you have a 98% Approval, I charge 2%, because you are higher risk. It works like that down to 95%. Anything below that and you have to call The General.
